Grant Report Template
All organizations that receive grants from Community Foundation Grey Bruce are required to submit a Report Form with receipts to support the use of the grant money. Photos of the project are also required (with the exception of organizations whose clients are at-risk).
Failure to submit a Final Report will result in non-eligibility for future grants. If the project is delayed, please contact the Grants Co-ordinator to discuss the options 519-371-7203.
- Report deadline for Spring Grants - February 1st of following year
- Report deadline for Fall Grants - July 1 of following year
Final Reports for both Comunity Grants and Capital Grants must be completed online through the granting portal
Go to the granting portal to complete an online Final Report - click here. When you reach the dashboard, scroll to the bottom to find "Follow Ups". You will find the Final Grant Report here. Access it by clicking on "edit". With the Report, each organization will be required to upload a Grant Expense form. If you would ike to fill the expense form out as you go, you may download the form below to your computer. **note - this form is not fillable online, it must be downloaded to a computer, filled out, saved, then upload in the Report Form
Consent form for Photo, video or voice recording
Community Foundation Grey Bruce wants to ensure privacy of their community members. To this end, the Foundation needs to be sure anyone in a photo or video provided by the grantee has had the consent of the persons in the image. Please download and submit a Consent Form to accompany any photo, video or recording that has recognizable persons in the image so that the Foundation can safely present your photos or videos.
